What are Ketones in the Urine and above 7%?

Sorry about all these questions but after my doctors appointment and just having coffee with Nutrasweet and my BG was 259 he also mentioned something in my urine referring to Something that started with an H? but he then said keytones? is this correct and what does this mean if mine is over 9% after just finding out I am Diabetic like my father and grandmother who went blind from it, I am afraid I might go blind since thats one real obvious symptom that my BG's are high in the 200's. Many thanks you all are a gods send! I have taken the Glucotrol by the books my wife makes me eat food that wouldn't fill up my parakeet! I feel very heavy and withdrawn and just want to sleep but end up wakening to urinate every hour or two. Thanks for everyone's input! Keytones is when your kidneys spill sugar. It's an indication of sugars that are way too high. Getting this under control like you are trying to do is the best way to avoid the complications. Don't starve yourself though! That can actually make things worse. Eat sensible and low carb. Have you been to a diabetes education class? It's an excellent starting point. It teaches a lot about the different foods and what portioning is all about. The diet may need adujust accoriding to your needs but it's a basic. It's a lot to learn and it takes time. Doug... Do you have a meter? If not, get one. It is the most important weapon you can have in this fight. Also, you don' t have to eat like a parakeet... more like a mountain lion. Less carbs. Here's the advice I give all newbies: Sounds like you're planning a move to take control of your diabetes... good for you. There is so much to absorb... you don't have to rush into anything. Begin by using your best weapon in this war, your meter. You won't keel over today, you have time to experiment, test, learn, test and figure out just how your body and this disease are getting along. The most important thing you can do to learn about yourself and diabetes is test test test. The single biggest question a diabetic has to answer is: What do I eat? Unfortunately, the answer is pretty confusing. What confounds us all is the fact that different diabetics can get great results on wildly different food plans. Some of us here achieve great blood glucose control eating a high complex carbohydrate diet. Others find that anything over 75 - 100g of carbs a day is too much. Still others are somewhere in between. At the beginning all of us felt frustrated. We wanted to be handed THE way to eat, to ensure our continued health. But we all learned that there is no one way. Each of us had to find our own path, using the experience of those that went before, but still having to discover for ourselves how OUR bodies and this disease were coexisting. Ask questions, but remember each of us discovered on our own what works best for us. You can use our experiences as jumping off points, but eventually you'll work up a successful plan that is yours alone. What you are looking to discover is how different foods affect you. As I'm sure you've read, carbohydrates (sugars, wheat, rice... the things our Grandmas called "starches") raise blood sugars the most rapidly. Protein and fat do raise them, but not as high and much more slowly... so if you're a T2, generally the insulin your body still makes may take care of the rise. You might want to try some experiments. First: Eat whatever you've been currently eating... but write it all down. Test yourself at the following times: Upon waking (fasting) 1 hour after each meal 2 hours after each meal At bedtime That means 8 x each day. What you will discover by this is how long after a meal your highest reading comes... and how fast you return to "normal". Also, you may see that a meal that included bread, fruit or other carbs gives you a higher reading. Then for the next few days, try to curb your carbs. Eliminate breads, cereals, rices, beans, any wheat products, potato, corn, fruit... get all your carbs from veggies. Test at the same schedule above. If you try this for a few days, you may find some pretty damn good readings. It's worth a few days to discover. Eventually you can slowly add back carbs until you see them affecting your meter. The thing about this disease... though we share much in common and we need to follow certain guidelines... in the end, each of our bodies dictate our treatment and our success. The closer we get to non-diabetic numbers, the greater chance we have of avoiding horrible complications. The key here is AIM... I know that everyone is at a different point in their disease... and it is progressive. But, if we aim for the best numbers and do our best, we give ourselves the best shot at heath we've got. That's all we can do. Here's my opinion on what numbers to aim for, they are non-diabetic numbers. FBG under 110 One hour after meals under 140 Two hours after meals under 120 or for those in the mmol parts of the world: Fasting Under 6 One hour after meals Under 8 Two hours after meals Under 6.5 Recent studies have indicated that the most important numbers are your "after meal" numbers. They may be the most indicative of future complications, especially heart problems. Listen to your doctor, but you are the leader of your diabetic care team. While his /her advice is learned, it is not absolute. You will end up knowing much more about your body and how it's handling diabetes than your doctor will. Your meter is your best weapon. Just remember, we're not in a race or a competition with anyone but ourselves... Play around with your food plan... TEST TEST TEST. Learn what foods cause spikes, what foods cause cravings... Use your body as a science experiment. You'll read about a lot of different ways people use to control their diabetes... Many are diametrically opposed. After awhile you'll learn that there is no one size fits all around here. Take some time to experiment and you'll soon discover the plan that works for you. Best of luck! It could be that you are eating the wrong types of foods. Carbs are what raise the BG. You still need to eat some. And how many you can eat is very much an individual thing. For now, you need to eat less carbs than you've been eating. Increase the non-starchy vegetables and the protein. As the others have said, eating less or skipping meals will only cause your liver to dump glucose and drive your BG even higher. Part of the problem is that your BG is so high. And when it is that high, your body is essentially starving, no matter what you eat. The high BG is preventing the food you eat to get into your cells. Get that BG down and your appetite will decrease. You can plan on feeling really yucky for a week or two until your BG has come down and gotten stable. Thanks for everyone's input! I'm new to this, too, and I also take Glucotrol, which is keeping me in control. I've read that ketones are really two acids and a ketone. The acids are called hydroxybutyric and acetoacetic. That may be the "h" mentioned by your doctor. I found the following on the net, which explains ketones for me: In people with diabetes, glucose builds up in the blood instead of going into cells. Your body gets rid of some of this extra glucose makes your urine thick. So as a result your body pulls fluid from all over to thin down the urine. You make lots of urine, and you are very thirsty. Too-high levels are bad in the short term. Without glucose, your cells starve. As a result, you may feel tired or thirsty or hungry. When blood sugar levels are very high, the starved cells start breaking down body fat for energy. This process creates chemicals called ketones. Ketones can make you sick. Doug, Trying to piece together what your doc *probably* said from what *you* said: 1. You had a fasting blood glucose (FBG) of 259. Normal is 70-120. 2. You probably had ketones in your urine (although you might have protein). Assuming it is ketones, this is a waste product of burning fat. Our bodies burn fat when they cannot get enough nutrition from glucose. Since your glucose was high, you have diabetes. 3. An indirect measure of blood glucose over the past 30-90 days is called glycated hemoglobin or HbA1c. This is often shortened to just A1c. This number should be approx. 4.0-6.5% (labs vary). The net result of #1, 2, 3 above is you are diabetic. All 3 measures are currently high, but *this is not uncommon in the recently diagnosed*. The good news is that you are not gonna keel over tomorrow. The bad news is that you need to do something about it ASAP. Unless your wife knows a lot about nutrition, it is unlikely that you are eating the right diet for a diabetic yet. The best thing you can do right now, is cut back on the carbs, esp. things like bread, pasta, rice, potatoes, etc. They are the foods that raise your BG the most. Don't cut carbs to zero. Try cutting your current consumption in half to start. Call your doc and get a meter and strips prescription immediately. Get whatever make and model your insurance will pay for. If you don't have insurance, get a "Relion" meter and strips from Wal-Mart. Also, see if your doc can refer you to a diabetes training class at your local hospital. Increase your exercise. This can be as simple as a half-hour walk everyday to start. Jennifer will probably post her "welcome" to newbies for you. It contains a distillation of the advice from this newsgroup on starting out with diabetes. If you are impatient, click the URL in my sig, and then select "Newbies" from the left menu. No insurance, that's something I'm familiar with. 1. Use Walmart's Relion meter for $9 and 100 sticks for $42. Cheapest on the market. EBay has some good prices on sticks, but I've yet to see any price that beats the Relion sticks. 2. Your doctor is probably charging you whatever the market will bear. The cost of the office visit is substantially above what the office visit would cost with insurance. Negogiate the price down. Your readings are way too high. They need to be brought under control. Diet, exercise, and in your case, meds will probably do the trick. IIRC you have a lot of other health problems too. All of this can cause BG's to go out of whack. Few questions: Do you ask for a copy of your lab reports? This is important to have so you know what tests have been ordered and what the results are. You have a legal right to them so don't let the doctor's tell you it isn't important. If you need to see other specialists the records can save time and guesswork on the part of doctors. Do you do Web searches for information? Researching diabetes on the web can be very helpful. A lot of information is written for the lay-person in easy to read language. It really helps you to get a sense of the whys and wherefores of this illness. Do you have flexibility regarding choice of doctors? There are good doctors out there who accept Medicare/Medicaid patient. They aren't plentiful but they are out there. If you have some flexibility in choice it is important to find a doctor you feel comfortable with. A good doctor will answer your questions regarding your medical tests and condition. However, you can't expect the doctor to manage you diabetes for you. That's the job of both of you. Prayers, Doug Yes, Glad you mentioned that Colleen, I have heard so many different opinions on this, but every time I call the SSD office they tell me I'm not eligible until August of this year which is 2 years from the date of filing for SSD. Yeah, you qualify for Medicare after two years on SSD. Have you applied for Medicaid? That is the other program for low income disables folks. Call social services and see if you qualify. It's worth a shot. Medicare covers doctors for me and Medicaid covers medication. I am going to test after every meal and before and right down what I have eaten and its carbs and sugar content etc. my wife actually was going to go to school to be a nutritionist (spell?) but unfortunately we decided on having kids and have two beautiful kids a girl and a boy, so it halted her ambition for school, well she spent a lot of money on this diabeties book diet that the store only had one left and it covers every aspect of a diabetics diet. And yes it is a lot to absorb over night but I will take it one day at a time. my doctor is switching my glucotrol to something else,? My BG after fasting was 259? isn't that high to only take one tablet in the AM and one at PM? its through the county since I'm on SSD and have no insurance, I thought he would take a more aggressive approach? my father takes 4 different pills throughout the day for his diabeties, Is it because the county wants to keep cost of meds down? should I see a diabetic specialist doctor or a real doctor per say? many thanks to everyone, you all are very helpful! Doug My Medicaid will only pay doctor bills over 615.00? So I try to tell the doctor to do some xrays and lab work to get my total over 600. so I'm not stuck with the bill! Now this is me only on SSD raising 2 kids by myself since my wife and I have recently split apart, so from my wages and what I get for the kids Medicare will only pay 615. or more? I do not understand how they think me paying the mortgage and utilities and clothes for the kids which my wife does help there but Medicare should be more considerate since I am flat broke and to embarrassed to get on food stamps again. my kids have a piece of toast and peanut butter for breakfast with milk that cost a fortune, the highlight of there day is free school lunches! thats pretty sad! and they go to bed hungry because of our wonderful government helping other countries before there own! Oh! No.... We are suppose to go wait in lines at church missions meals on wheels for the street people? I'm sorry, but I would rather starve than eat that garbage, my wife helps me with my diabetic diet, she will pick me up a salad and cheese and high protein items like peanut butter and sunflower seeds. and I keep the portions very small!. I think it is swallowing my pride and think about my kids and screw the lines I hold up paying for there food with food stamps! God must have directed you to send that post because I was thinking a lot of it is true!
